Psycho-Social Well-being Support

A structured, gentle program to support confidence, routine, & participation

For some people, physical challenges are only part of the picture. Changes in confidence, routine, sleep, mood, and social connection can have a significant impact on daily life, independence, and overall well-being for people living with disability or long-term health conditions.

A group of friends in their community

Our Psycho-Social Well-being Support program is designed to gently support these areas alongside physical health. Delivered over a structured 12-week period, this program focuses on building practical skills, confidence, and habits that support everyday life at a pace that feels achievable and supportive.

This program is most commonly accessed through the NDIS and is introduced only when it is likely to provide meaningful benefit to the individual and their family.

    Who this program is for

This program may be suitable for NDIS participants who are experiencing:

    • Reduced confidence or motivation
    • Difficulty maintaining routines or daily structure
    • Sleep disruption or fatigue
    • Anxiety or emotional regulation challenges
    • Social withdrawal or reduced community engagement
    • Difficulty sustaining healthy lifestyle habits

It is particularly helpful for individuals whose well-being has been impacted by neurological conditions, long-term disability, or significant changes in function or independence.

    How the program works

At Best Physiotherapy, we believe psycho-social support should be practical, respectful, and tailored to the individual. It should not be overwhelming or prescriptive.

    Here’s what you can expect:

Understanding the individual’s strengths, challenges, routines, and goals.

A clear but flexible framework that progresses gradually over time.

Support delivered in familiar, comfortable environments.

The program adapts as confidence, needs, and goals change.

Clear communication with families and support coordinators where appropriate.

The aim is not to “fix” someone, but to support meaningful, sustainable change.

Areas of focus within the program

The program is built around key areas that support overall well-being and participation:

Supporting consistent routines, improved sleep habits, and energy management.

Encouraging safe, enjoyable movement as part of everyday life.

Developing skills to recognise, manage, and respond to emotional stressors.

Practical strategies to reduce overwhelm and build resilience.

    Supporting realistic, sustainable lifestyle choices.

Gradual support to reconnect with interests, activities, and community roles.

Each area is addressed at a pace that suits the individual.

    Our approach

    • Gentle and structured
    • Strengths-based and respectful
    • Focused on real-world application
    • Adapted to the individual’s capacity and goals

We prioritise dignity, encouragement, and achievable progress, rather than rigid expectations.

    NDIS Funding and Access

This program is commonly funded under NDIS Capacity Building supports. We work closely with participants, families, and support coordinators to ensure:

    • Goals align with the NDIS plan
    • Supports are clearly documented
    • Progress is communicated appropriately

If you’re unsure whether this program fits within a plan, our team is happy to discuss options.

    When is this program introduced?

This program is not for everyone and it doesn’t need to be.

    It is typically recommended when:

    • Progress has plateaued with physical supports alone
    • Psycho-social factors are limiting participation or independence
    • Additional structure would benefit confidence and routine
    • Families are seeking more holistic, long-term support

We will always discuss suitability carefully before commencing.
